Important Notes:
• Student applies to undergraduate, graduate and medical students and post-doctoral research trainees.
• Paramedical applies to psychologists, dieticians, social workers, laboratory technicians and professionals
  active in disciplines with interest gender medicine
• All applications for registrations with deductions (Residents/Nurses, Paramedical and Students) must be
  accompanied by a letter from their respective Heads of the Department.
• Applications for registration with deductions for members of National Societies for Gender
  Medicine must be accompanied by a letter from the president/secretary of the respective society.

Please complete the online Registration Form and send it together with your payment to the Congress Secretariat. Payments can be made online, by bank draft or wire transfer to account #142-472330,
IBAN : IL58-0127-8000-0000-0472-330, Bank Hapoalim (swift code poalilit), Branch no. 780, Itzhak Sade’ Street, Tel Aviv or by Credit Card (Visa/MasterCard only).

Online registration and payment by credit card are on a secure server.

Personal checks are not accepted! Payment by Credit Card will be charged in US$, based on the exchange rate between the Euro and the US$ on the date of payment.

On Site Registration

There will be an on site registration desk at the David Intercontinental Hotel, the Congress venue, throughout the Congress. However, availability of the hotel accommodation and participation to social activities may not be guaranteed. Cash ($US or Euros) and credit card payment will be accepted.

Payment Terms

Congress Registration
Registration fees will be charged upon receipt of registration forms. Payments will be refunded according to the cancellation policy.

Tourist Services
Two nights deposit will be charged upon receipt of your “Registration Form”. The rest of the Tourist Services reservation balance will be charged three weeks prior to your arrival.
Payments will be refunded according to the cancellation policy.

Cancellation Policy
Refund of registration fees or Tourist Services payments will be made if written notification of cancellation is received at the Congress Secretariat as follows:

• Before October 12, 2010 -  Full refund less € 90
• From October 13, 2010 -  No refund

Please Note! You can purchase a Cancellation Insurance for 10% of your total balance (non refundable), which allows you to receive a full refund of registration and all Tourist Services payments (including accommodation, tours and transfers) with a notification of up to 48 hours prior to the Congress commencement. If you purchase this insurance, the cancellation fees above will not apply.

Important Notes!
• Registration forms received without payment will not be processed.
• Each registrant should complete a separate registration form. 
• Written confirmation will be sent upon receipt of registration form and full payment.
• The Congress program is subject to change without prior notice.
• All inquiries, changes and cancellations should be made in writing to Ortra Ltd, the Congress Secretariat.

Additional Tourist Services Reservations
Special requests for additional tourist services reservations, such as hotel extensions, regular tours, private car tours, car rentals, touring in limousines with driver/guide, etc. can be booked through the Congress Secretariat and Official Travel Agent, Ortra.
